10 Steps to Complete to Receive Academic Adjustments for a Disability

1. Indicate the need for academic adjustments.
2. Read and review disability support services handout.
3. Complete Tri-State University Disability Support Services Application form.
4. Sign Consent for Release of Information.
5. Provide adequate documentation from a professional source.
6. Qualify for academic adjustments.
7. Complete conference with Academic Support Services.
8. Complete Academic Adjustment Request Form each semester.
9. Sign Authorization for Release of Information.
10. Schedule appointments with all professors during the first two weeks of the semester.
