Directions: You will be considered for placement as a Tri-State University student teacher when all forms described below have been submitted. Final placement is contingent upon School and Teacher Education Committee approval.

1. STUDENT TEACHING APPLICATION
You are to complete this form. It must be typed. Make certain there are no mistakes. Have it proofread by someone you know will spot any errors. You may use liquid paper to make corrections as the form will be copied before it is sent. It will be sent to schools/teachers during the placement process. It is the first glimpse of you anyone will have, so make sure it is perfect. Return the completed form to the Franks School of Education office.

2. RECOMMENDATION FORMS
Two of these forms must be submitted. At least one must be completed by a member of the TSU faculty—someone who has had you as a student in a class. Whoever recommends you should send the form directly to the Franks School of Education office. Your only responsibility is to select the individual and give him/her the form. If you submit the form directly, it will not be accepted.

3. GPA VERIFICATION FORM
You must have at least an overall 2.5 GPA and at least a 2.5 GPA in your major area(s) of study before you will be considered for student teaching. This form is to be completed and signed by your academic advisor. The advisor will submit it directly to the Franks School of Education office. Your responsibility is to deliver the form to your advisor.

4. LIMITED CRIMINAL HISTORY INFORMATION FORM
You need to request that a limited criminal history check be completed by the Indiana State Police. You are to send the appropriate form (Request For Limited Criminal History Information) and money order to the Indiana State Police. A paper with the seal of the Indiana State Police will be returned to you. You are to submit that official paper to the Franks School of Education office. A limited criminal history check is valid for one year. If you have a form on file that is valid, you do not need to request a new criminal background check. Forms are also available online.

5. PLACEMENT REQUEST FORM
You are to complete this form. DO NOT MAKE SCHOOL DISTRICT AND/OR TEACHER CONTACTS YOURSELF. The Franks School of Education will make any contacts that are necessary. Return the completed form to the Franks School of Education office.
