Federal Stafford loan requirement

Entrance Interview Form

First Time Borrowers of Stafford Loans must complete this form. If you are applying for a federal student loan you are required to complete an entrance interview form. You may complete your loan entrance interview form on-campus during orientation or electronically by clicking here.

Exit Interview Form
Borrowers of Stafford Loans who will be graduating or will be enrolled less than half time must complete this form. This form is a federal requirement to review your rights and responsibilities as a Stafford Loan Borrower. You may complete this form by clicking here.
